About The Position
As a Nurse Practitioner in Internal Medicine, you’ll provide comprehensive primary care to adult patients in an outpatient setting. You’ll work alongside a collaborative care team to manage acute and chronic conditions, promote preventive care and build lasting relationships with your patients. The position is in Primary Care at the Wilson Multicultural Medical Campus in Rochester, New York.
Requirements
- Master of Science in Nursing (required).
- Nurse Practitioner certification through the NYS Office of Professions.
- Registered Nurse license through the NYS Office of Professions.
- Strong customer service and communication skills.
- Compassionate, warm, and patient-focused approach.
- Ability to work in a stressful environment and take appropriate action.
- Critical thinking skills, decisive judgment, and ability to work with minimal supervision.
- Exceptional documentation skills and professional behavior.
Responsibilities
- Provide comprehensive primary care to adult patients, including evaluation, diagnosis, and management of acute and chronic conditions.
- Develop treatment plans, prescribe medications, order and interpret diagnostic testing, and provide preventive care and patient education.
- Collaborate with physicians, APPs, and other healthcare team members to coordinate care and specialty referrals as needed.
- Maintain timely and accurate documentation of patient visits, diagnoses, and treatment plans.
