About The Position

As a Nurse Practitioner in Internal Medicine, you’ll provide comprehensive primary care to adult patients in an outpatient setting. You’ll work alongside a collaborative care team to manage acute and chronic conditions, promote preventive care, and build lasting relationships with your patients. The position is full time at Bay Creek Medical Campus in the Primary Care - Internal Medicine department, with a Monday–Friday day schedule.

Requirements

Master of Science in Nursing (required).
Nurse Practitioner certification from the NYS Office of Professions.
Registered Nurse license from the NYS Office of Professions.
Strong customer service and communication skills.

Responsibilities

Provide comprehensive primary care to adult patients, including evaluating, diagnosing, and managing acute and chronic conditions.
Develop treatment plans, prescribe medications, order and interpret diagnostic testing, and provide preventive care and patient education.
Collaborate with physicians, APPs, and other healthcare team members to coordinate care and specialty referrals as needed.
Maintain timely and accurate documentation of patient visits, diagnoses, and treatment plans.
