About The Position

Physician Solutions Group is hiring a full-time Certified Registered Nurse Practitioner (CRNP) in York, Pennsylvania. As part of the clinical team, the Nurse Practitioner uses advanced knowledge and a diverse skill set to assess the healthcare needs of chronically ill geriatric members in collaboration with physicians and interdisciplinary teams. The role focuses on helping members maximize their quality of life and live as independently as possible in their homes and communities. The position does not require weekends, evenings, or holidays.

Requirements

• At least one year of experience working with an elderly community.
• Strong passion for helping the geriatric population.

Responsibilities

• Perform health histories and in-depth medical examinations for new members to determine preliminary Plans of Care.
• Initiate, evaluate, and review medical orders, and identify opportunities to improve care.
• Carry out procedures and document care and recommendations.
• Order, interpret, and follow up on laboratory tests and results.
• Request consultations from medical and non-medical in-house and external providers, including physicians, therapists, dietitians, social workers, and other specialists.
• Educate staff, members, caregivers, and families on essential care functions to help members maintain independence at home and in centers.
• Complete ongoing assessments and determine Plan of Care changes or improvements based on member feedback and medical evaluations.
