About The Position

WellBe Senior Medical delivers value-based care directly in patients’ homes. The Nurse Practitioner provides field-based primary care to seniors, with autonomy and support from an interdisciplinary care team. The role focuses on whole-person, outcomes-based care, including addressing social determinants of health and managing a dedicated patient panel. The position is located in Albany, New York, and includes local field-based travel. Salary is $110,001–$175,088, plus mileage and full benefits; relocation is available. Employment is contingent on completing a pre-employment drug screening and background check. WellBe conducts monthly FACIS checks on employees, and ongoing employment may be subject to drug testing under the company’s Drug-Free Workplace Policy. Applicants must be legally authorized to work in the United States without current or future employer visa sponsorship. Employment is at-will unless otherwise specified by contract.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license.
Board certification through ANCC or AANP.
Valid driver’s license and vehicle for local field-based travel.
Must be legally authorized to work in the United States without employer visa sponsorship now or in the future.
Successfully complete a pre-employment drug screening and background check.

Responsibilities

Conduct comprehensive primary care visits and physical exams in patients’ homes.
Create longitudinal care plans to manage chronic diseases in high-risk patients.
Partner with an interdisciplinary team to support seamless, safe care transitions.
Treat acute needs in the home to help prevent emergency room visits and hospitalization.
