About The Position

WellBe Senior Medical delivers value-based care directly in patients’ homes. The Nurse Practitioner provides field-based primary care to seniors, with autonomy and support from an interdisciplinary care team. The role includes comprehensive home visits, chronic disease management, coordination of care transitions, and treatment of acute needs to help patients stay safe and avoid unnecessary hospital visits. The position is based in Albany, New York, and includes local field travel. Salary is $110,001–$175,088, plus mileage and full benefits; relocation is available. The organization supports clinicians with daily team check-ins, ongoing training, and one-to-one mentorship. Employment requires successful completion of pre-employment drug screening and a background check. Employees are also subject to monthly FACIS checks and applicable drug-free workplace testing. WellBe does not provide employment-based visa sponsorship.

Requirements

Master’s degree as a Nurse Practitioner.
Active, unencumbered Nurse Practitioner license.
Board certification through ANCC or AANP.
Valid driver’s license and access to a vehicle for local field-based travel.
Legally authorized to work in the United States without current or future employer visa sponsorship.
Successful completion of a pre-employment drug screening and background check.

Nice To Haves

Responsibilities
