About The Position

Humana’s Primary Care Organization is seeking a Nurse Practitioner to join its CenterWell office in Burlington, North Carolina. This is an onsite float position with a variable, PRN schedule, requiring coverage across multiple office locations in the Charlotte area. The role reports to the Associate Medical Director and provides comprehensive outpatient care to adult and senior patients in a value-based, team-based care setting.

Requirements

• Master’s degree in Nursing or completion of a Physician Assistant program with board certification.
• Active, unrestricted Nurse Practitioner or Physician Assistant license in the state of practice.
• At least 2 years of Nurse Practitioner or Physician Assistant experience in a primary care setting.
• Passion for value-based care and integrated team-based models.
• Basic computer proficiency, including EMR and email.
• Willingness to cover multiple office locations throughout the Charlotte area.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in an on-call rotation.
• Cover multiple office locations throughout the Charlotte area.
