About The Position

Humana's Primary Care Organization is a senior-focused, value-based care provider operating CenterWell and Conviva centers. The Nurse Practitioner will join the CenterWell office in Burlington, North Carolina, delivering outpatient care to adult and senior patients. This is an onsite float position with a variable, PRN schedule, requiring coverage at multiple office locations throughout the Charlotte area. The role reports to the Associate Medical Director and includes participation in an on-call rotation.

Requirements

• Master's degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Minimum of 2 years of NP or PA experience in a primary care setting.
• Passion for value-based care and integrated team-based models.
• Basic computer proficiency, including EMR and email.
• Willingness to cover multiple office locations throughout the Charlotte area.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in an on-call rotation.
• Cover multiple office locations throughout the Charlotte area.
