About The Position

Community Care is hiring a full-time Nurse Practitioner for its PACE Program at its Racine/Kenosha and Milwaukee locations. The position supports the organization’s mission to help older adults and adults with physical and intellectual disabilities live as independently as possible in the community. It involves team collaboration and community nursing case management. The Nurse Practitioner documents and updates members’ health information and evaluations, provides guidance and leadership to interdisciplinary team staff, assists with care planning, and educates members and families. Regular local travel using a personal vehicle is required.

Requirements

• Master’s degree in Nursing.
• Wisconsin Registered Nurse/RN license.
• Family, Geriatric, or Adult Nurse Practitioner certification.
• DEA registration.
• Advanced Nurse Prescriber credential.
• NPI and CPR and AED credentials.
• Wisconsin driver’s license.
• At least 1 year of recent clinical nursing experience with an at-risk or elderly population.

Responsibilities

• Document and update health history and findings, physical exams, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
• Within the official scope of practice, delegate appropriate tasks to interdisciplinary team staff and provide leadership and guidance.
• Serve as a resource to staff to support appropriate care and follow-through, and provide care-planning assistance.
• Educate members and families about health maintenance and acute and chronic illnesses.
• Travel locally on a regular basis using a personal vehicle; maintain a valid driver’s license and proof of required insurance coverage.
• Fulfill on-call responsibilities for 6–7 weeks per year.
