About The Position

The Nurse Practitioner organizes, plans, implements, evaluates, and provides patient care in various hospital and clinic settings, following hospital and nursing standards. The role supports patients with chronic or challenging wounds and contributes to disease-state management, wellness, and safe health-program operations.

Requirements

• Licensure from an accredited school.
• Current BLS or ALCS certification.
• Strong communication skills.
• Exceptional time-management skills.
• Ability to work efficiently and respond quickly.
• Accurate and proper handling of lab work.

Nice To Haves

• Bilingual in English and Spanish.

Responsibilities

• Order and schedule necessary studies and professional consultations.
• Collect and document data.
• Provide direct patient care, including prescribing medications and other therapies as needed.
• Conduct physical examinations, make diagnoses, and determine courses of treatment as permitted by certification and state Standards of Practice.
• Provide chronic care management and patient education when needed.
• Serve as a consultant for health-program operations involving disease-state management, adverse health trends, and safety concerns.
• Help arrange appropriate diagnostic testing.
• Assist with treatment decisions alongside other clinical team members, including disease-management and wellness coaches.
