•$130,000 - $150,000•Hybrid

About The Position

DaVita Integrated Kidney Care is hiring a Nurse Practitioner to provide holistic, integrated care to patients with complex kidney conditions in Tacoma, Washington. The position is hybrid, combining occasional telehealth work from home with travel across an assigned geographic area, including dialysis clinics and nephrology practices. Travel expectations may vary with business needs and patient population. The primary clinical responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions and address patients’ medical, social, emotional, and financial needs. The Nurse Practitioner manages patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD), with a focus on slowing disease progression, reducing costs and hospitalizations, and preventing readmissions. The role uses prescriptive authority, medication management, and diagnostic interpretation in collaboration with nephrologists and interdisciplinary teams. It also leads the clinical team in coordinating care with medical treatment plans and addressing medical and psychosocial needs. This is a full-time, Monday–Friday position with a self-managed schedule that accommodates patient and nephrology-partner availability. There are no nights, weekends, or on-call duties. DaVita describes the role as offering autonomy to lead and innovate in patient care as part of a mission-driven, collaborative team.
