About The Position
This full-time mobile Nurse Practitioner role provides personalized healthcare in offices, healthcare facilities, and patients’ homes. The clinician delivers vascular care management, works collaboratively with a dedicated healthcare team and an Ultrasound Tech, and helps improve access to care for diverse patient populations. Local travel is essential, with approximately 75% travel throughout the service area.
Requirements
- Current and unrestricted Nurse Practitioner license.
- Active DEA and CDS licenses.
- Current BLS and ACLS certifications.
- Valid driver’s license and reliable transportation.
- Willingness and ability to travel regularly throughout the service area.
- Strong clinical assessment and diagnostic skills.
- Knowledge of healthcare regulations, quality standards, and patient care best practices.
- Exceptional communication and patient education abilities.
- Ability to work independently while thriving in a collaborative team environment.
- Excellent critical thinking, problem-solving, and decision-making skills.
- Commitment to ongoing professional development and lifelong learning.
Nice To Haves
- Experience practicing as an Advanced Practice Nurse in primary care, hospitals, outpatient clinics, emergency medicine, or related healthcare environments; new APN graduates are also considered.
- Bilingual English/Spanish skills.
Responsibilities
- Perform comprehensive patient assessments, physical examinations, and medical history reviews.
- Order, interpret, and evaluate diagnostic testing, including laboratory studies and imaging.
- Diagnose and treat acute and chronic vascular conditions.
- Develop, implement, and manage individualized treatment plans.
- Prescribe medications and monitor treatment effectiveness.
- Educate patients and caregivers on health conditions, preventive care, and treatment plans.
- Collaborate with referring providers and interdisciplinary care teams to ensure seamless patient care.
- Maintain accurate and timely clinical documentation.
- Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
