About The Position

Aligned Modern Health (AMH) is seeking outstanding Nurse Practitioners to join their Functional Medicine practice. AMH is the largest evidence-based holistic medicine practice in the Midwest, operating 16 clinics across Chicago with a rapidly expanding telehealth practice serving patients nationwide. Their collaborative team provides personalized care across functional medicine, hormone health/BHRT, clinical nutrition, chiropractic care, rehabilitation and massage therapy, and acupuncture. AMH focuses on targeting the root causes of symptoms and chronic conditions through advanced diagnostics, expert-led care, and individualized treatments, empowering lasting well-being. The organization is committed to improving the healthcare landscape and is looking for licensed Nurse Practitioners with experience in Functional Medicine and conducting functional diagnostic testing.

Requirements

At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs
Experience in functional and integrative medicine settings preferred
NP/APRN – with a concentration in Functional Medicine, Hormone Health, Anti-Aging or Lifestyle Medicine
Providers based in FL, MI, TN, and TX are preferred
Must have (or be able to obtain) an IL APRN license and any remote location determined
Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers
Energetic and outgoing personality with a desire to help people
Patient first attitude and ability to consistently provide a 5-star level of service
Knowledge of HIPAA regulations and ability to maintain patient confidentiality

Nice To Haves

Active IL APRN license would be required to start.
Providers based in FL, MI, TN, and TX, are highly preferred.

Responsibilities

Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
Develop personalized treatment plans based on patients' needs and medical conditions
Prescribe nutraceuticals and/or medications and monitor their effectiveness
Collaborate with other healthcare professionals to coordinate patient care
Provide counseling and education to patients on disease prevention and healthy lifestyle choices
Maintain accurate and up-to-date patient records using electronic health record systems
Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine
