Nurse Practitioner (PRN) - In-Home Health Assessments

Matrix Medical Network•Oxford, MS
•Onsite

About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home Comprehensive Health Assessments (CHAs) for adult and geriatric members. Visits can last up to one hour and involve reviewing and observing members’ current health, medical history, medication adherence, social environment, and other risks. These assessments provide insight into members’ overall health and may help close multiple care gaps during one visit. Services are provided at members’ residences, including private homes and Skilled Nursing Facilities. The role offers scheduling flexibility: visits are arranged according to provider availability and are conducted Sunday through Saturday, from 8 a.m. to 7 p.m. Providers earn per completed assessment, with the stated rate depending on state of residence and monthly earnings depending on the number of visits completed. Matrix provides onboarding, a preceptor, individual feedback, and access to continuing education. If business needs exist, Matrix may support additional state licensure and credentialing. PRN providers may transition to full-time positions.
