About The Position

We are seeking a Nurse Practitioner who will work collaboratively within a multidisciplinary health team to optimize team performance and patient outcomes and is responsible for providing comprehensive and age-appropriate care to both well and/or ill patients. The Nurse Practitioner educates patients and/or intervenes in acute or chronic illness. The Nurse Practitioner practices within the guidelines of the unit department, and institution and in accordance with the Nurse Practitioner- Physician Written Agreement required and approved by the State of Maryland.

Requirements

  • Master's Degree in Nursing
  • Registered Nurse license
  • Certified as a Nurse Practitioner
  • Must be licensed as NP in the State of Maryland
  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.

Responsibilities

  • Use advanced clinical and theoretical knowledge to identify acute and/or chronic health problems through a comprehensive, age-appropriate assessment of patients/families.
  • Provide evidence-based care and provides patient education for self-care chronic disease management.
  • Effectively use resources. Prioritize and organize work to meet the changing priorities of the functional unit and/or cost center.
  • Coordinate and make decisions regarding patient care within scope of authority.
  • Triage phone calls and electronic advice requests from patients and determine need for medical assessment.
  • Assess lab results and implement intervention in consultation with primary providers and medical director.
  • Provide guidance and direction to other unit staff where applicable to promote quality, safety, and excellence in care.
  • Analyze findings and apply conclusions, as appropriate, to improve the management of patient care.
  • Identify care continuity needs of patients.
  • Participate in quality improvement/patient safety projects.
  • Coordinate care for a group of primary complex patients to achieve clinical and organizational outcomes.
  • Other clinical duties as assigned.
