Nurse Practitioner - F/T - Hazelwood Florissant Internal Medicine
About The Position
Esse Health is currently seeking a full-time Nurse Practitioner for our Hazelwood Florissant Internal Medicine office located at 637 Dunn Road in Hazelwood. This is an outpatient-focused clinical practice where you will join a collaborative team and develop a 1:1 relationship with patients and clinical staff members. You will provide comprehensive primary care services to adult patients, conduct patient evaluations, physical exams, and diagnostic assessments, and diagnose and manage acute and chronic conditions. You will develop and implement treatment plans in collaboration with a physician partner, emphasizing preventive care, wellness, and patient education. Accurate and efficient documentation in the electronic health record (NextGen) is required, as is coordinating care with specialists and other healthcare team members. Participation in quality improvement and care coordination initiatives is also expected.
Requirements
- Master’s Degree of Science in Nursing
- Current Nurse Practitioner licensure in Missouri (or eligibility)
- Certified Nurse Practitioner (or eligibility)
- Strong clinical, communication, and interpersonal skills
- Commitment to patient-centered, team-based care
- Ability to work independently while collaborating closely with physicians
