The Nurse Practitioner provides value-based medical care and care coordination for homebound patients under the supervision of the Managing Physician for Primary Care at Home. The role supports planning, implementation, and evaluation of medical and nursing care for patients with complex needs. The Care Manager assesses patients’ physical and psychosocial conditions through comprehensive or problem-focused examinations and medical histories; orders, collects, interprets, and evaluates laboratory and other diagnostic tests; and may prescribe or recommend drug therapies and other treatments under approved protocols and policies, considering efficacy, safety, and cost. The position develops and implements outcomes-based care plans informed by patient assessments, clinical rationale, standards of care, and professional practice guidelines, and initiates consultations or referrals when issues exceed the practitioner’s scope or expertise. The position requires home and virtual visits, with a goal of approximately six home visits per day. The practitioner also serves as a physician extender in the clinic setting and completes clinic evaluations, documentation, ordering, follow-up, referrals, and other clinical tasks within scope and as directed by the supervising physician.
