About The Position

In collaboration with a physician, diagnoses and treats certain acute or chronic health conditions in specific populations. This role involves obtaining patient history, performing assessments, ordering and interpreting diagnostic studies, and providing direct treatment and management of health conditions. The Nurse Practitioner will also counsel and educate patients and their families on preventative health, treatment options, and community resources, ensuring care is provided in a manner that promotes age and culture-appropriate informed choices, shared decision-making, and the right to self-determination. Accurate and timely documentation of patient care plans is essential. The role may also involve coordinating registration functions, including collecting demographic and insurance information, and processing co-payments.

  • Master's degree or higher in Nursing required upon hire.
  • RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire.
  • APRN - Licensed as an Advanced Practice Registered Nurse in the state of Kentucky required upon hire.
  • APRN - National certification from American Academy of Nurse Practitioners (AANP) required upon hire OR APRN - National certification from American Association of Critical Care Nurses (AACCN) required upon hire OR APRN - National certification from National Certification for the Obstetric, Gynecologic, and Neonatal Nursing Specialties (NCCCOGN) required upon hire OR APRN - National certification from Pediatric Nursing Certification Board (PNCB) required upon hire OR APRN - National certification from American Nurses Credentialing Center (ANCC) required upon hire OR APRN - National certification as Neonatal Nurse Practitioner from National Certification Corporation (NCC) required upon hire.

Responsibilities

  • Obtains patient history and performs assessment via observation, interview, and examination.
  • Orders, performs, and interprets diagnostic studies.
  • May be required to perform preventative health assessments, screenings, immunizations, and care.
  • Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
  • Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options, and community resources.
  • Provides care in the context of promoting age and culture-appropriate informed choices, shared decision-making, and the right to self-determination.
  • Provides accurate and timely documentation of assessment and patient care plan.
  • May coordinate the registration functions including collecting and entering demographic, insurance, employer information, and co-payments.
