Requirements

  • Master's degree
  • Graduation from an accredited school of nursing, with Michigan licensure to practice as a nurse practitioner or physician assistant.
  • Minimum 2 years of job-related experience.
  • Staff supervision experience.
  • One to two years of nursing experience required.
  • Effective verbal and written communication skills.
  • Ability to build and maintain working relationships with internal and external partners.
  • Demonstrated problem solving and critical thinking skills.
  • Strong organizational, time management and planning skills.
  • Ability to be flexible and adaptable to change.

Nice To Haves

  • BSN preferred.
  • Related research experience in designated area of work preferred.

Responsibilities

  • Provides outpatient clinical care for patients and coordinates services and activities within the Wayne State University Adult HIV program.
  • Collaborates with physicians, nurses, medical assistants, social workers, medical case managers, patient advocates and peer navigators as a member of the interdisciplinary care team.
  • Develops and ensures compliance with program policies and data collection.
  • Leads quality improvement projects within the program and conducts ongoing program evaluation.
  • Supports re-linkage to care initiatives for persons out of care or lost to care.
  • Conducts home visits or off-site care as needed or specified within programs scope of services.
  • Develops innovative care models to increase viral suppression among patients, with emphasis on collaborative care model and team building and maintenance as well as strong networking skills.
  • Performs appropriate clinical care of patients in the program in the clinic, off site and in the home as needed, in accordance with the standards of the clinic and federal guidelines.
  • Documents care appropriately in the medical record.
  • Develops and implements program evaluation systems and quality improvement projects in accordance with funder and program needs.
  • May participate in clinical research.
  • Conducts outreach among other appropriate practices within and outside of Wayne State University and Wayne Health to assess needs and support patients.
  • Connects with HIV care service providers to ensure coordination of services.
  • Attends local and regional meetings or trainings related to public health and infectious diseases.
  • Perform other duties as required.
