Seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to join their team in delivering high-quality care to our diverse and underserved patient population. Requirements

• Graduate of an accredited NP or PA program
• Current unrestricted APRN or PA license
• National board certification & BLS certification
• DEA registration
• Interest in value-based care and population health

Responsibilities

• Provide comprehensive primary care services to patients of all backgrounds, including preventive, wellness, and acute care visits.
• Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines.
• Collaborate with nurses, care coordinators, behavioral health providers, and specialists to manage care transitions, ensuring continuity, particularly for medically complex patients.
• Build trusting, long-term relationships through effective, empathetic communication.
• Educate patients and families on disease management and healthy lifestyle changes.
• Utilize Electronic Health Record (EHR) systems to maintain accurate, up-to-date, and detailed documentation in the electronic medical record system.
