The company's technology currently serves over 40,000 post-acute patients daily across various networks. This role involves transforming patient care by utilizing AI at the bedside and in the home. The Nurse Practitioner will work across skilled nursing and Home Health AI Care programs, conducting patient assessments, acting on predictive health signals, and coaching facility teams to implement earlier interventions that reduce preventable readmissions. Responsibilities include rounding on new admits and flagged patients in SNF facilities, and providing proactive care in patients' homes within the home health setting. The role blends direct patient care with technology integration through training, demonstrations, and assessments.

Requirements

• Certified and licensed Nurse Practitioner or Physician Assistant in MI; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP/PA experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Nice To Haves

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

Responsibilities

• Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Complete documentation/charts within the same day.
• Efficiently complete visits across designated SNFs (typically 30+/day where appropriate) and in patient homes for our Home Health program (7+/day), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Surface field insights to improve workflows and tech integration in a clinical setting.
• Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Cover SNF sites and patient homes within a 30–40 mile radius. For locations outside that radius, mileage and lodging are covered per company policy.
