About The Position

Bluestone Physician Services is hiring a Nurse Practitioner or Physician Assistant to provide exceptional, team-based primary care to patients living with complex, chronic conditions and disabilities in Assisted Living and Memory Care communities in and around Minneapolis/St. Paul, Minnesota. Care is delivered on site and tailored to patients’ needs through collaboration with patients, families, healthcare providers, and multidisciplinary colleagues. The organization uses an evidence-based approach, quality care management, and data-driven medical decisions to support chronic condition management, address social determinants of health, coordinate transitions to and from inpatient settings, and provide behavioral health support. Providers manage a panel of approximately 200 patients, typically seeing 12–16 patients per day, with an average of 14. Full-time providers care for patients Monday through Thursday, with the option to use Fridays as a clinical or administrative day. Providers can design and manage their schedules and receive clinical support from Clinical Assistants and care coordination staff, including LPNs, RNs, and Social Workers. Diagnostics such as labs and X-rays are available as needed. Providers also collaborate with community partners, including home care, hospice, and other resources. Compensation is base pay plus quality/productivity incentives, with a stated range of $125,000–$170,000.
