About The Position

Infinite Medical P.C. is hiring a full-time Primary Care Nurse Practitioner or Physician Assistant to provide on-site primary care to residents in skilled nursing and long-term care facilities in Rome, New York. Requirements

Hold an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
Hold a state-specific license in good standing in New York.
Have at least 6 months of experience in long-term care or a skilled nursing facility.
Have strong communication, clinical, and critical-thinking skills.
Have a passion for elder care and person-centered healthcare.

Responsibilities

Provide primary care services to residents in skilled nursing and long-term care facilities.
Perform physical examinations and medical history assessments.
Diagnose and treat chronic and acute health conditions.
Order and interpret diagnostic tests and laboratory results.
Develop and adjust individualized care plans.
Prescribe medications and monitor treatment effectiveness.
Collaborate with facility staff and interdisciplinary teams.
Educate patients and families about treatment options and care goals.
Ensure a safe, respectful, and supportive environment for residents.
