About The Position

Atrium Health is seeking experienced Nurse Practitioners and Physician Assistants to join their Atrium Health Transition Services Team in the greater Charlotte area. The Transition Clinic is an outpatient post-discharge clinic focused on helping patients restore their body and mind after hospitalization. It is a multidisciplinary ambulatory Internal Medicine clinic designed for high-risk patients who may experience complications or readmissions due to medical complexity or barriers to healthcare access. The clinic offers integrated visits with a provider, pharmacist, and social worker. During a 30-day follow-up period, the team identifies and alleviates barriers to health, emphasizing patient engagement, access, and education. Care can be delivered in-office, virtually, or with community paramedicine support, depending on the patient's needs. The role is 100% outpatient, with office hours Monday-Friday from 8 am to 5 pm. There is a shared, phone-only call rotation, covered one week at a time, with no nights or weekends required. The ideal candidate will have an interest in Internal Medicine, seeing patients 18 years and older, a passion for providing quality patient care to complex medical patients, and a focus on multidisciplinary collaboration. Experience is preferred.
