About The Position

The Wilmot Cancer Institute Survivorship Program is seeking a Nurse Practitioner or Physician Assistant to join their multidisciplinary comprehensive breast survivorship team. This role involves identifying and tracking appropriate patients, reviewing treatment summaries and follow-up care plans with patients and families, providing individualized resources for survivors, offering sexual wellness care, triaging phone calls, managing urgent add-on clinic visits, and conducting scheduled ambulatory long-term follow-up clinics. The position requires excellent communication skills, independent judgment, initiative, flexibility, dependability, compassion, and a passion for this patient population. Strong assessment and multi-tasking abilities are important for success. Requirements

• Master's degree as a Nurse Practitioner (for NP role)
• Active New York State Registered Nurse license in good standing (for NP role)
• Active New York State Nurse Practitioner licensure in good standing (for NP role)
• Current national certification as a Nurse Practitioner (for NP role)
• Completion of an ARC-PA approved Physician Assistant program (for PA role)
• Active New York State Physician Assistant licensure in good standing (for PA role)
• Current national certification as a Physician Assistant by NCCPA (for PA role)

Responsibilities

• Identification and tracking of appropriate patients
• Review with patients and families the cancer care treatment summary and follow-up care plans
• Provide a comprehensive, holistic summary of individualized resources to help survivors
• Providing sexual wellness care
• Triaging phone calls
• Managing urgent add on clinic visits
• Conducting scheduled ambulatory long-term follow up clinics
• Participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training.
