About The Position
Alignment Health is committed to serving seniors and the chronically ill and frail. The Field Remote Nurse Practitioner/Physician Assistant will be part of the Care Anywhere (CAW) program, designed to provide care and support to Alignment Healthcare patients in their homes. This program aims to prevent unnecessary hospitalizations, health complications, and unmanaged disease progression by providing timely clinical interventions. Requirements
- Master’s degree from an accredited NP Program or PA program.
- Ability to communicate positively, professionally, and effectively with others.
- Effective problem-solving, organizational, and time management skills.
- Ability to work in a fast-paced environment.
- Active Nurse Practitioner or Physician Assistant license for the state of California.
- Active Nurse Practitioner Board Certification/Physician Assistant Certification.
- NPI Number, DEA, Valid BLS.
- Valid Driver License and current automobile insurance.
Nice To Haves
- One (1) year of prior clinical or home care experience.
- Previous EMR experience.
- Experience in the care of older adult (geriatric) patients.
Responsibilities
- Conduct in-home assessments on Alignment members, medication review, and health screenings three days a week.
- Work from home two days a week.
- Provide patient education by matching desired care with the best available care.
- Identify diagnoses for assessment in care management and active medical management.
- Partner and communicate with the Regional Medical Officer and other team members to develop appropriate care plans.
- Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers.
- Lead broader clinical teams, which may include nurses, health coaches, social workers, and care coordinators.
