About The Position

Bluestone Physician Services is hiring a Nurse Practitioner or Physician Assistant to provide team-based primary care to patients living with complex, chronic conditions and disabilities. The role brings care directly to patients in Assisted Living and Memory Care communities in and around Woodbury, Minnesota. Bluestone’s multidisciplinary model emphasizes preventative, proactive, individualized care, chronic condition management, transitions between inpatient and community settings, social determinants of health, and behavioral health support.

Providers manage a panel of approximately 200 patients, typically seeing 12–16 patients per day. Full-time providers care for patients Monday through Thursday, with the option for Friday to be a clinical or administrative day. Providers can design and manage their schedules and receive support from Clinical Assistants, LPNs, RNs, and Social Workers, with access to diagnostics such as laboratory tests and X-rays. They collaborate with community partners, including home care and hospice providers.

Compensation is listed as $125,000–$170,000, with base pay plus quality and productivity components. Requirements

• Graduate of an accredited Nurse Practitioner or Physician Assistant education program.
• Board certified or board prepared through AANP, ANCC, or NCCPA.
• Licensed, or able to obtain licensure, through the Board of Nursing or Board of Medical Practice.
• Able to obtain and/or maintain DEA certification for the state of work.
• Proven ability to communicate effectively, with strong verbal skills.
• Strong interpersonal and customer service skills.
• Valid and unrestricted driver’s license and minimum state-required liability auto insurance.

Responsibilities

• Provide on-site, team-oriented primary care to patients in Assisted Living and Memory Care communities.

• Travel locally to patient communities.
