About The Position

Atrium Health is seeking experienced Nurse Practitioners and Physician Assistants to join their Atrium Health Transition Services Team in the greater Charlotte area. The Transition Clinic is an outpatient post-discharge clinic focused on helping patients restore their body and mind after hospitalization. It is a multidisciplinary ambulatory Internal Medicine clinic designed for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access. The clinic offers integrated visits with a provider, pharmacist, and social worker. During the 30-day follow-up period, the team identifies and alleviates barriers to health, focusing on patient engagement, access, and education. Care is delivered in-office, virtually, and/or with community paramedicine support based on patient needs. Atrium Health emphasizes a team effort in providing high-quality medical care and addressing health barriers.

Requirements

• Experienced Nurse Practitioner or Physician Assistant.
• Interest in Internal Medicine.
• Ability to see patients 18 years old and up.
• Passion for providing quality patient care to complex medical patients.
• Focus on multidisciplinary collaboration.

Responsibilities

• Provide outpatient post-discharge care to patients.
• Work in a multidisciplinary ambulatory Internal Medicine clinic.

• Participate in a shared phone-only call rotation.
