About The Position

Optum Home & Community Care, part of the UnitedHealth Group family, is building an integrated care model to address patients' physical, mental, and social needs. The Senior Community Care (SCC) product team provides care to patients in their homes, nursing homes, and assisted living facilities. This role is part of the SNF Patient Connect program, focused on reducing hospital readmissions, improving documentation, closing quality measure gaps, increasing Advanced Care Planning, and enhancing patient/family satisfaction. The practitioner will conduct a one-time comprehensive health assessment in collaboration with case management to improve care coordination and health outcomes. This is an autonomous role offering significant professional satisfaction.

Requirements

  • Active and unrestricted license in the state which you reside
  • Certified Nurse Practitioner through a national board; or Physician Assistant through accredited program
  • For NPs: Graduate of an accredited master’s degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP), with preferred certification as ANP, FNP, or GNP; or for PA's - Physician Assistant National Certification through NCCPA.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Driver’s license and access to reliable transportation that will enable you to travel to SNF facilities within a designated area
  • Ability to move a 30-pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations
  • Availability to work 10 hours per week
  • Ability to gain a collaborative practice agreement, if applicable in your state

Nice To Haves

  • 1+ years of hands-on post grad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including the ability to document medical information with written and electronic medical records
  • Demonstrated ability to develop and maintain positive customer relationships
  • Proven adaptability to change
