About The Position

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join a well-established inpatient palliative care team. This is an excellent opportunity for a compassionate provider who is passionate about delivering high-quality, patient-centered care to individuals living with serious illness. You will work alongside an experienced interdisciplinary team, providing consultative and follow-up palliative care services in a collaborative hospital environment.

Requirements

  • Active New Jersey Nurse Practitioner or Physician Assistant license.
  • National board certification.
  • DEA and CDS registration, or ability to obtain before start date.
  • Minimum of 2 years of inpatient palliative care experience required.
  • Excellent communication, clinical judgment, and interpersonal skills.
  • Ability to work effectively within an interdisciplinary team.

Nice To Haves

  • Palliative care certification (ACHPN, CHPN, or equivalent) preferred.

Responsibilities

  • Provide inpatient palliative care consultations and follow-up visits.
  • Assess and manage complex symptoms for patients with serious or advanced illnesses.
  • Lead goals-of-care discussions and advance care planning with patients and their families.
  • Collaborate with physicians, nurses, social workers, care coordinators, and other healthcare professionals.
  • Coordinate patient transitions, discharge planning, and continuity of care.
  • Maintain accurate and timely clinical documentation.
  • Deliver evidence-based, compassionate, and patient-centered care.
