About The Position

The Nurse Practitioner, Palliative Care provides comprehensive, high-quality palliative care to new and established patients. The role includes conducting patient visits and coordinating care plans with the designated team in patients’ homes, facilities, and hospitals. The Nurse Practitioner makes independent decisions about patients’ healthcare needs and delivers palliative care services as required. The Medical Director is available for consultation, collaboration, and referrals.

Requirements

• Graduation from an accredited Baccalaureate school of nursing.
• Completed training in the diagnosis and management of common and some complex medical conditions.
• Current licensure in the State of California.
• Certification as an Adult, Family, Pediatric, or Women’s Health Nurse Practitioner.
• Previous experience with computer-based systems.
• A valid BLS certification or higher, following American Heart Association or American Red Cross guidelines.

Nice To Haves

• Experience with electronic medical records.
• Previous experience in hospice or palliative care, high-risk populations, including home-based care.
• Bilingual in Spanish and English.
