About The Position

Work Schedule: 100% FTE, day shift working Monday – Friday, 8a – 5p. You will work at the Mile Bluff, Lake Delton and Portage clinics. Pay: The pay range listed reflects both titles. We are seeking a Nurse Practitioner or Physician Assistant to participate in multidisciplinary HD rounds/ collaborate with the patient's care team for care coordination. This role involves writing and managing dialysis orders, managing and treating anemia and BMD, assessing and referring for dialysis access placement or intervention, blood pressure management, volume status/estimated dry weight assessment, lab results review/management, and referral for transplant and/or home dialysis program.

Requirements

  • Minimum – If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
  • If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
  • Minimum – State of Wisconsin NP or PA licensure
  • If Nurse Practitioner: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a NP in area of clinical practice. Adult; Adult/Gerontology Primary Care, Adult/Gerontology Acute Care.
  • All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire.
