About The Position

Mass General Brigham Community Physicians, Inc. is seeking a full-time, 40-hour Nurse Practitioner or Physician Assistant to join their Internal Medicine team. This position is based at their practice located at 260 Merrimac Street in Newburyport, MA. The practice is open Monday through Friday from 8:00 AM to 5:00 PM. The APP will provide 36 hours of direct patient care and 4 hours of dedicated administrative time each week. There are no weekend, holiday, or on-call responsibilities associated with this role. The APP will provide comprehensive primary care services to an adult patient population, managing a wide range of acute and chronic medical conditions. Responsibilities include conducting annual wellness visits, evaluating and treating acute concerns, managing chronic diseases, coordinating care with specialists, and promoting preventive health and wellness. Requirements

  • Master's Degree Nursing required
  • Advanced Practice Registered Nurse [APRN-NP - State License] - Generic - HR Only required
  • New graduate Nurse Practitioners should have prior RN experience.

  • Experience as a Nurse Practitioner 0-1 year preferred

Responsibilities

  • Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
  • Performs complete histories and physical examinations.
  • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
  • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
  • Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
  • Performs bedside procedures as are appropriate to the patient population.
  • Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
  • Assessing and managing various patient populations, including those with medical and/or surgical conditions.
  • Providing diagnostic and therapeutic interventions.
  • Developing appropriate plans of care.
  • Conducting ongoing evaluations.
  • Conducting annual wellness visits.
  • Evaluating and treating acute concerns.
  • Managing chronic diseases.
  • Coordinating care with specialists.
  • Promoting preventive health and wellness.
