About The Position

The Primary Care Provider (NP) has a specialized focus on perinatal and women's health. This includes preventive care, diagnosis, and treatment of various health conditions specific to gynecology, including reproductive health, pregnancy, menopause, gender affirming care, and general wellness. The provider will work collaboratively with other healthcare professionals to ensure each patient receives holistic and individualized care. The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health (AAAHC) and the Commission of Dental Accreditation (CODA).

Requirements

• Must be a licensed FNP in the state of California.
• Must be Board Certified in WFNP, Family Practice, Women's Health or other full scope Primary Care Specialty.
• Current DEA license.
• Three (3) years of experience working within a community clinic.
• Must be able to provide TB and Physical clearance prior to start date.
• CPR certification must be obtained within 90 days and kept current at all times.
• Meets credentialing requirements established by the Native American Health Center.
• Must have a National Provider Identification (NPI), BLS, DEA and depending on specialty ACLS or NALS or PALS or PEARS may also be required.
• Able to furnish Vaccination history prior to start date dependent on role.
• Excellent communication and written skills.
• Ability to work independently and as part of a multi-disciplinary team.
• Ability to work well under pressure, analyze and evaluate individual member’s needs, reach sound conclusions and make appropriate recommendations.
• Must be detail oriented and be able to multi-task effectively.

Nice To Haves

• Previous experience with an Electronic Health Records (EHR) system and working in an EHR environment is desired.
• Two to five (2-5) years of experience working with a community clinic is highly desirable.
• Excellent communication and written skills.
• Established leadership skills.
• Bilingual in Spanish.
• Experience with EPIC EHR system is strongly desired.

Responsibilities

• Provide comprehensive primary care with emphasis on women's health throughout the lifespan
• Perform preventive screenings, routine gynecological procedures including Pap smears, and health maintenance
• Manage acute and chronic conditions with attention to gender-specific presentations
• Deliver reproductive health services, including contraception counseling, gender affirming care, and LARC procedures including IUD and Nexplanon
• Address women's health concerns including menstrual disorders, menopause, and sexual health
• Implement evidence-based treatment plans incorporating physical, mental, and social health factors
• Lead daily huddles with complex care team including RNs, care coordinators, and specialists
• Champion NAHC's integrated care model through effective team leadership
• Mentor and develop Medical Assistants and clinical support staff
• Participate in interdisciplinary care planning and case conferences
• Maintain productive relationships with internal and external healthcare partners
• Complete clinical documentation within 72 hours of patient visits
• Process medication refills and authorization requests within 5 business days
• Complete patient medical forms within 10 business days
• Meet productivity standard of 18-20 patients per day
• Fulfill mandatory reporting requirements for births, deaths, and reportable conditions
• Maintain compliance with HIPAA regulations and clinic policies
• Participate in quality improvement initiatives and stay current with medical research
• Provide culturally-sensitive health education to patients and families
• Develop and implement health education programs
• Participate in community health initiatives
• Consider cultural, psychological, and environmental factors in treatment planning.
• Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves.
• Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.
• Keep all protected health information (PHI) confidential and abide by HIPAA guidelines for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to Site Director and HIPAA security officer.
• Ensure the protection of all PHI and compliance of HIPAA guidelines amongst staff under your supervision. Take appropriate steps to address HIPAA concerns with staff as necessary.

• At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
