About The Position

Hartford HealthCare’s Neighborhood Health program is seeking passionate per diem Advanced Practitioners to join our primary care team dedicated to improving health equity in underserved communities. This role offers the opportunity to provide high-quality preventative and urgent care through mobile and community-based services, addressing health disparities and promoting wellness. Join us in making a real impact on lives across Connecticut with a collaborative, patient-centered approach. This position requires travel to our home base hubs out of Hartford and New Britain then travel in company fleet to clinic sites and Street Medicine. The setting is 100% Outpatient Primary Care/Urgent Care – Family Medicine. This is a full-time position, weekdays only, with no call required.

Requirements

  • Unrestricted State of Connecticut Medical License (or eligible)
  • Family Nurse Practitioner (FNP) or Physician Assistant (PA) training
  • 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.
  • Valid driver’s License within 3 Months (90 days) of hire date or job transfer date
  • Ability to push/pull/lift 50 lbs

Responsibilities

  • Perform routine physical examinations under established guidelines and in collaboration and/or supervision with the practice physician(s).
  • Formulate medical, educational, and psychosocial plans to treat patients.
  • Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists, in consultation with the physician(s) and care team members.
  • Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
  • Identify problems outside of their scope of practice and refer appropriately to physicians.
