In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Cambridge MA
About The Position
Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will provide flexible, comprehensive, and personalized health evaluations directly to individuals. The evaluations typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. The clinician will use an iPad with an intuitive workflow and is not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, or airborne allergens. Candidates must be comfortable and safe performing duties in these diverse environments.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Essex and surrounding area

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with people in their homes to bridge gaps in care.
- Build trusted relationships to improve health outcomes.
- Provide flexible, comprehensive, and personalized health evaluations.
- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Administer diagnostic tests as ordered (e.g., diabetic eye exams, spirometry).
- Use an iPad and clinical workflow for evaluations.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services including Diagnostic & Preventive Services products.
- Fulfill the requirements of the specified role as a clinician colleague with Signify Health.
