About The Position

Esse Health is currently seeking a compassionate full-time Advanced Practice Provider (NP/PA) for our Hazelwood Florissant Internal Medicine office located at 637 Dunn Road in Hazelwood. This is a full-time opening, Monday - Friday, 8:00 a.m. - 4:00 p.m., with no nights, weekends, or holidays. The role is outpatient focused and offers the opportunity to join a collaborative team, develop a 1:1 relationship with patients and clinical staff, and provide comprehensive primary care services to adult patients. Responsibilities include conducting patient evaluations, physical exams, and diagnostic assessments, diagnosing and managing acute and chronic conditions, developing and implementing treatment plans in collaboration with a physician partner, emphasizing preventive care, wellness, and patient education, documenting patient care accurately and efficiently in the electronic health record (NextGen), coordinating care with specialists and other healthcare team members, and participating in quality improvement and care coordination initiatives.

Requirements

Master’s Degree of Science in Nursing or Physician Assistant-Master’s Degree
Current Nurse Practitioner or Physician Assistant licensure in Missouri (or eligibility)
Certified Physician Assistant or Certified Nurse Practitioner (or eligibility)
Strong clinical, communication, and interpersonal skills
Commitment to patient-centered, team-based care
Ability to work independently while collaborating closely with physicians
