About The Position

Esse Health is currently seeking a compassionate full-time Advanced Practice Provider (NP/PA) for our Hazelwood Florissant Internal Medicine office located at 637 Dunn Road in Hazelwood. This is a full-time opening with a Monday - Friday schedule, from 8:00 a.m. to 4:00 p.m., with no nights, weekends, or holidays. The role is outpatient focused and offers the opportunity to practice at the highest level of training within a physician-led, supportive practice environment. The provider will join a collaborative team and develop a 1:1 relationship with patients and clinical staff members, providing comprehensive primary care services to adult patients.

Requirements

Master’s Degree of Science in Nursing or Physician Assistant-Master’s Degree
Current Nurse Practitioner or Physician Assistant licensure in Missouri (or eligibility)
Certified Physician Assistant or Certified Nurse Practitioner (or eligibility)
Strong clinical, communication, and interpersonal skills
Commitment to patient-centered, team-based care
Ability to work independently while collaborating closely with physicians

Responsibilities

Provide comprehensive primary care services to adult patients
Conduct patient evaluations, physical exams, and diagnostic assessments
Diagnose and manage acute and chronic conditions
Develop and implement treatment plans in collaboration with physician partner
Emphasize preventive care, wellness, and patient education
Document patient care accurately and efficiently in the electronic health record (NextGen)
Coordinate care with specialists and other members of the healthcare team
Participate in quality improvement and care coordination initiatives
