About The Position

Join a growing team of geriatrics providers at Hartford Healthcare and make a meaningful impact in the lives of geriatric patients! We are seeking a full-time Nurse Practitioner or Physician Assistant for our home-based REACH (Resource for Elderly and Care at Home) Program, delivering compassionate care in the comfort of patients’ homes and assisted living facilities. This role is open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA) and is based in the greater Manchester, CT area. The setting is Geriatric Medicine in the home setting, working within a multi-disciplinary care team. The schedule is full-time, weekdays only. Hartford HealthCare supports Nurse Practitioners and Physician Assistants in practice with dedicated support and voice through the Office of Advanced Practice directed by Sarah Clark, MBA, MHS, PA-C, BSN, RN, System Director of Advanced Practice. Requirements

  • Unrestricted State of Connecticut Medical License (or eligible)
  • APRN (Family, Adult/Geri Nurse Practitioner) or PA
  • At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) in Geriatric Medicine or a related field

Responsibilities

  • Perform routine physical examinations.
  • Formulate medical, educational, and psychosocial plans to treat patients.
  • Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
  • Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
  • Identify problems outside of their scope of practice and refers appropriately to physicians.
