Nurse Practitioner - Geriatrics
About The Position
As a Nurse Practitioner in Geriatrics, you will provide comprehensive primary and acute care to older adults across nursing home, assisted living, and independent living settings. You will manage a variety of acute and chronic conditions and build meaningful, long-term relationships with patients. The role is part of an experienced, collaborative geriatrics team with strong clinical support and an engaged onboarding experience. This is a full-time position in the Rochester area with a Monday–Friday daytime schedule and no weekends or call. Care is delivered in collaboration with physicians, nursing, facility staff, and other members of the interdisciplinary team.
Requirements
- Master of Science in Nursing (MSN).
- Nurse Practitioner certification through the NYS Office of Professions.
- Registered Nurse license through the NYS Office of Professions.
- Compassionate, patient-focused approach to caring for older adults.
- Strong clinical assessment, communication, and decision-making skills.
- Ability to work collaboratively across multiple care settings.

- Medicine or geriatric experience.
Responsibilities
- Provide primary and acute care to older adults, including assessment, diagnosis, treatment, medication management, and follow-up.
- Manage acute and chronic conditions across nursing home, assisted living, and independent living settings.
- Partner with physicians, nursing, facility staff, and other care-team members to coordinate comprehensive patient care.
- Monitor patient progress, adjust treatment plans as appropriate, and maintain timely and accurate documentation.
