About The Position

As a Physician Assistant in Geriatrics, you’ll provide comprehensive care to older adults across nursing home, assisted living, and independent living settings. You’ll manage a variety of acute and chronic conditions while developing meaningful, long-term relationships with patients. You’ll join an experienced and collaborative geriatrics team that offers strong clinical support and an engaged onboarding experience. This is a full-time position based at nursing homes and assisted living facilities throughout the Rochester area. The schedule is days, Monday through Friday, with no weekends or call.

Requirements

Bachelor of Science in Physician Assistant studies (required).
Certification as a Physician Assistant (PA-C) through the National Commission on Certification of Physician Assistants (NCCPA), or Registered Physician Assistant (RPA) through the NYS Office of Professions.
Compassionate, patient-focused approach to caring for older adults.
Strong clinical assessment, communication, and decision-making skills.
Ability to work collaboratively across multiple care settings.

Nice To Haves

Medicine or geriatric experience.

Responsibilities

Provide primary and acute care to older adults, including assessment, diagnosis, treatment, medication management, and follow-up.
Manage acute and chronic conditions across nursing home, assisted living, and independent living settings.
Partner with physicians, nursing, facility staff, and other members of the care team to coordinate comprehensive patient care.
Monitor patient progress, adjust treatment plans as appropriate, and maintain timely and accurate documentation.
