About The Position

Esse Health is seeking a compassionate Advanced Practice Provider (NP/PA) for our Hazelwood Florissant Internal Medicine office. This role offers a full-time or part-time schedule, with the office open to discussing the type of schedule you are looking for. The office operates within regular business hours, Monday through Friday, 8:00 a.m. to 5:00 p.m., with no nights, weekends, or holidays. The position focuses on an outpatient clinical practice, emphasizing a collaborative team environment and direct patient relationships. Providers will deliver comprehensive primary care services to adult patients, including evaluations, physical exams, diagnostic assessments, and the management of acute and chronic conditions. A key aspect of the role involves developing and implementing treatment plans in collaboration with a physician partner, with a strong emphasis on preventive care, wellness, and patient education. Accurate and efficient documentation in the electronic health record (NextGen) is essential, as is coordinating care with specialists and participating in quality improvement initiatives.

Requirements

Master’s Degree of Science in Nursing or Physician Assistant-Master’s Degree
Current Nurse Practitioner or Physician Assistant licensure in Missouri (or eligibility)
Certified Physician Assistant or Certified Nurse Practitioner (or eligibility)
Strong clinical, communication, and interpersonal skills
Commitment to patient-centered, team-based care
Ability to work independently while collaborating closely with physicians
Prefer 1+ years experience as a Nurse Practitioner or Physician Assistant

Responsibilities

Provide comprehensive primary care services to adult patients
Conduct patient evaluations, physical exams, and diagnostic assessments
Diagnose and manage acute and chronic conditions
Develop and implement treatment plans in collaboration with physician partner
Emphasize preventive care, wellness, and patient education
Document patient care accurately and efficiently in the electronic health record (NextGen)
Coordinate care with specialists and other members of the healthcare team
Participate in quality improvement and care coordination initiatives
