About The Position

The Advanced Practice Provider works to support the mission and vision of Neighborhood Healthcare (NHC) by providing excellent patient-centered care and services within a dynamic team environment. This role will provide general medical care and treatment to patients under the direction of providers. In addition, this role will assess training opportunities to promote team development.

Requirements

• Master's degree in nursing, required (for Nurse Practitioner)
• California Nurse Practitioner License in good standing, required (for Nurse Practitioner)
• Registered Nurse License in good standing, required (for Nurse Practitioner)
• Current CA DEA licensure is required and is to be maintained as a condition of employment
• Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment.
• Bachelor’s degree and completion of an accredited Physician Assistant Program (for Physician Assistant)
• California State Physician Assistant License in good standing (for Physician Assistant)
• Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
• Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
• Knowledgeable about and experience with providing direct care to patients
• Ability to successfully manage multiple tasks simultaneously
• Excellent planning and organizational ability
• Ability to work as part of a team as well as independently
• Ability to work with highly confidential information in a professional and ethical manner
• Ability to lift/carry 20 lbs/weight
• Ability to stand for long periods of time

Responsibilities

• Performs comprehensive physical examinations on patients, including review of patient medical data, such as health history
• Orders, administers, and interprets test results for diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
• Performs therapeutic procedures such as injections, immunizations, suturing, and wound care, and managing infection
• Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance
• Impacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracy
• Assesses and provides age-appropriate care to patients
• Shares accountability for overall patient health outcomes, working in coordination with care teams
• Operates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders
• Responds promptly and with caring actions to patients and employees
• Maintains professional working relationships with all levels of staff, clients, and the public
• Cooperates in accomplishing communicated department goals and objectives
• Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care
• Performs in accordance with NHC practice protocols and under the guidance and supervision of the supervising MD(s), according to the terms of the Supervisory Agreement
• Keeps informed of organizational activities and promote mission and goals
• Functions at highest level according to credentials and competency
• Follow all safety procedures as outlined in Neighborhood Healthcare’s Illness and Injury Prevention Plan (IIPP) and report any injuries and/or unsafe conditions immediately
• Maintain current knowledge of policies and procedures as they relate to safe work practices
• Follow all safety procedures and report unsafe conditions
• Use appropriate body mechanics to ensure an injury free environment
• Be familiar with location of nearest fire extinguisher and emergency exits
• Follow all infection control procedures including blood-borne pathogen protocols

• Comply with all regulations regarding corporate integrity and security obligations
• Report all behavior and/or activity that are unethical, fraudulent, or unlawful
