About The Position

The Advanced Practice Provider (APP) functions under the clinical supervision of licensed physicians, provides diagnostic and therapeutic health care to patients in Skilled Nursing, Memory Care, Assisted Living, Independent Living, Inpatient Rehabilitation Hospitals, and Inpatient Psychiatric Hospital environments.

Requirements

Preferred one year experience in Internal Medicine
Excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
Problem-solves and uses resources well independently and as part of a team.
Willingness to work in a fast-moving, high-growth work setting.
Knowledge of Electronic Health Record (EHR), e-prescribing software, scheduling software, and telemedicine software.
Master of Science- Nursing
Doctorate of Nurse Practice
Bachelor of Health Science in Physician Assistant studies
Valid Wisconsin Advanced Practice Nurse Practitioner License or
Valid Wisconsin Physician Assistant License
Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
National Commission of Certification of Physician Assistants (NCCPA)
Basic Life Support for the Healthcare Provider (BLS)

Responsibilities

Provide general medical care to various and/or specific patient levels- adults/geriatric.
Perform comprehensive evaluations including history and physical exams.
Address both acute and chronic primary care complaints, and able to ascertain medical urgency.
Establish and document reasonable medical diagnoses.
Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom.
Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
Accept feedback and direction from the supervising physician.
Create and implement a medical plan of care.
Prescribe medications as appropriate.
Perform timely documentation and billing.
Maintains confidentiality of all patient records.
