About The Position

The Advanced Practice Provider (APP) functions under the clinical supervision of licensed physicians, provides diagnostic and therapeutic health care to patients in Skilled Nursing, Memory Care, Assisted Living, Independent Living, Inpatient Rehabilitation Hospitals, and Inpatient Psychiatric Hospital environments. This role involves providing general medical care to adults/geriatric patients, performing comprehensive evaluations, addressing primary care complaints, establishing diagnoses, ordering diagnostic tests, and creating and implementing medical plans of care. The APP will also prescribe medications, perform timely documentation and billing, and maintain patient confidentiality.

Requirements

  • Preferred one year experience in Internal Medicine

  • Knowledge of Electronic Health Record (EHR), e-prescribing software, scheduling software, and telemedicine software.
  • Master of Science- Nursing OR Doctorate of Nurse Practice OR Bachelor of Health Science in Physician Assistant studies
  • Valid Wisconsin Advanced Practice Nurse Practitioner License OR Valid Wisconsin Physician Assistant License
  • Certification by the American Nurses Credentialing Center (ANCC) OR American Academy of Nurse Practitioners (AANP) OR National Commission of Certification of Physician Assistants (NCCPA)
  • Basic Life Support for the Healthcare Provider (BLS)

Responsibilities

  • Provide general medical care to various and/or specific patient levels- adults/geriatric.
  • Perform comprehensive evaluations including history and physical exams.
  • Address both acute and chronic primary care complaints, and able to ascertain medical urgency.
  • Establish and document reasonable medical diagnoses.
  • Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom.
  • Apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
  • Accept feedback and direction from the supervising physician.
  • Create and implement a medical plan of care.
  • Prescribe medications as appropriate.
  • Perform timely documentation and billing.
  • Maintains confidentiality of all patient records.
