Nurse Practitioner - Hospice (PRN)
About The Position
Elara Caring is seeking a committed and compassionate Hospice Nurse Practitioner to provide hospice and palliative care. The role includes completing routine and emergency face-to-face recertification visits for hospice patients, including admissions, transfers, and extended stays. The Nurse Practitioner assesses patient needs, documents findings, and collaborates with physicians and the interdisciplinary care team to manage symptoms and support the Plan of Care. The position provides bedside palliative care, education, and physical and emotional support to patients and families during crises. It also involves triaging needs by acuity to address care needs in the home and help prevent avoidable hospitalizations. The Nurse Practitioner maintains patient records and clinical documentation, billing, and coding in accordance with CMS and regulatory guidelines; coordinates quality care with the Medical Director, attending physicians, branch leadership, and the interdisciplinary team; trains and educates clinical staff; and helps develop clinical practice guidelines and standards for high-quality palliative care. This is a PRN position.
Requirements
- Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
- Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board.
- At least 1 year of clinical nursing experience.
- Maintain active DEA certification if state allows prescription of controlled substances by APPs.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
Nice To Haves
- At least 1 year of post-graduate clinical nursing experience in the hospice and palliative care field.
- Prior experience in Family Medicine, Geriatrics, and psychiatry.
Responsibilities
