Requirements
- Minimum 6 months experience in SNF/LTC or hospital/outpatient cardiology setting
- Active and unrestricted Nurse Practitioner license
- BLS/CPR certification
- Master’s degree in Nursing from an accredited NP program
- Strong communication, organizational, and time management skills
Nice To Haves
- DEA license Recommended
- Preferred cardiology experience (training available)
Responsibilities
- Provide on-site cardiology consultations including initial and follow-up evaluations
- Assess and manage cardiovascular conditions such as hypertension, CAD, heart failure, arrhythmias, and valvular disease
- Order, review, and interpret EKGs, echocardiograms, labs, and diagnostic studies
- Collaborate with cardiologists and interdisciplinary teams to develop treatment plans
- Manage and titrate cardiac medications per evidence-based guidelines
- Educate patients and caregivers on disease management and lifestyle modifications
- Ensure accurate EMR documentation and regulatory compliance
- Participate in quality improvement initiatives to reduce hospitalizations
