Requirements

  • Minimum 6 months experience in SNF/LTC or hospital/outpatient cardiology setting
  • Active and unrestricted Nurse Practitioner license
  • BLS/CPR certification
  • Master’s degree in Nursing from an accredited NP program
  • Strong communication, organizational, and time management skills

Nice To Haves

  • DEA license Recommended
  • Preferred cardiology experience (training available)

Responsibilities

  • Provide on-site cardiology consultations including initial and follow-up evaluations
  • Assess and manage cardiovascular conditions such as hypertension, CAD, heart failure, arrhythmias, and valvular disease
  • Order, review, and interpret EKGs, echocardiograms, labs, and diagnostic studies
  • Collaborate with cardiologists and interdisciplinary teams to develop treatment plans
  • Manage and titrate cardiac medications per evidence-based guidelines
  • Educate patients and caregivers on disease management and lifestyle modifications
  • Ensure accurate EMR documentation and regulatory compliance
  • Participate in quality improvement initiatives to reduce hospitalizations
