The Advanced Practitioner Role Scope includes Nurse Practitioners (APRN) / Physician Assistants providing advanced heart failure consultations within the Skilled Nursing Facility setting throughout the East Region as well as scheduled time in the advanced heart failure outpatient clinic located in Norwich, CT. Responsibilities include assessing newly admitted heart failure patients to the SNF setting, collaborating with other providers on Cardiac Care with the goal of reducing hospital readmissions, and assisting in coordinating discharge by providing bridge appointments in the outpatient clinic.

Requirements

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program
  • Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut
  • Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
  • Cardiology experience required
