About The Position

The Advanced Practitioner (APP) will be dividing their time between SNF settings. The APP will play a vital role in filling the gaps and needs of our heart failure population, educating our patients and families on lifestyle modifications and GDMT, and providing an additional resource to the affiliated cardiology practices. Requirements

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut.
  • Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse.
  • Cardiology experience required.

Nice To Haves

  • Geriatric board certification preferred.
