About The Position

The Colorado Coalition for the Homeless is hiring a primary care provider to deliver high-quality care to people currently or formerly experiencing homelessness in the Denver metro area. The provider will work in fast-paced clinical environments and use trauma-informed care, motivational interviewing, and harm-reduction principles to partner with patients on their health goals. The role involves 2–3 days per week at the Recuperative Care Center, including one weekend day, and 1–2 days per week in other CCH programs, including Stout Street Health Center, the mobile Health Outreach Program, and satellite clinics. The position is primarily located at 2130 Stout Street and 2175 California Street in Denver and may require travel to other clinical sites in the CCH network. This is a full-time, 40-hour-per-week position with one weekend day (Saturday or Sunday) as part of the regular schedule. Schedules are typically either four 10-hour shifts or five 8-hour shifts per week. Typical 10-hour shift hours are 7:00 AM–5:00 PM, with Wednesday hours of 8:00 AM–6:00 PM. After the first 6–12 months with CCH, the provider is expected to take two weeks of at-home call per year; call is by phone only, with no in-person requirement. The role requires a master's or doctoral degree in nursing from an appropriately accredited program, Colorado advanced practice nurse registration or ability to obtain it, nurse practitioner certification, and current DEA licensure. Applicants must have at least two years of clinical practice experience. The posting also describes educational and licensure requirements for Physician Assistant applicants, although the listed job title is Nurse Practitioner/APN. The work may take place in outpatient clinics, mobile clinics, street outreach programs, and indoor or outdoor environments. Physical demands include extended standing and walking, lifting or assisting patients or equipment, hands-on medical tasks, and adapting to long shifts and weekend work. The role may involve exposure to heat, cold, extreme weather, noise, uneven or slippery surfaces, close proximity to others, offensive odors, and moving objects or vehicles.
