About The Position

The Nurse Navigator works in an organized and time-conscious manner and uses effective customer service and interpersonal skills. The role serves as a liaison among patients, professional staff, and physicians, providing care management and coordination for adult and pediatric patients with complex conditions, social needs, or education needs. Working collaboratively with primary care providers, specialists, other members of the health care team, and patients and families, the Nurse Navigator helps ensure quality, efficient, and cost-effective care. The position assesses, plans, implements, coordinates, monitors, and evaluates care options and services to optimize patients’ health and reduce hospital and emergency room utilization. It also integrates evidence-based clinical guidelines and protocols into individualized, patient-centric care plans that promote quality and efficiency.

Requirements

Currently licensed as a Registered Nurse in the State of Michigan.

Nice To Haves

Bachelor of Science degree in nursing preferred.
Minimum 5 years of experience as an RN is highly preferred.
